What is MiPCT?

MiPCT stands for the Michigan Primary Care Transformation project

 
Goals of the MiPCT project:
The main goal of the MiPCT project is to improve the delivery of primary care and lower health care costs while demonstrating a budget neutral system of care.  During this 3 year period, the services will only be offered to a select number of patients whose insurances are participating in the project.  If the project is a success, it is likely that many other insurances will cover these services and we will be able to offer this to many more of our patients.  Other goals of the project are:
  • Reduce unjustified health care usage and expenditures
  • Increase the ability of beneficiaries to participate in decisions concerning their care
  • Improve the safety, effectiveness, timeliness, and efficiency of health care
  • Increase the availability and delivery of care that is consistent with evidence-based guidelines in underserved areas
   

The Care Manager

 
The MiPCT project utilizes the role of a Care Manager.  A Care Manager possesses a unique skill set that will help meet the goals of the MiPCT project.  Each Care Manager is selected by the practice ensure the best patient care possible.  A Care Manager can help with the following:
  • Work with your physician to create a personalized care plan for you
  • Work to connect you with community resources and get you the assistance you need
  • Help facilitate and coordinate your care
  • Provide support and education to help you understand your care plan and take steps to a healthier life
